DC switch issues

We have two major issues going on with an install.

Problem #1.
The voltage is reading 16-18 volts on the power supply. The readout on the input voltage says 24.6 volts. What would be causing the voltage loss?

Problem #2
I logged into the switch tonight and all the POE ports said errors. Now I can't get into the switch. If I disable and re-enable the SFP port on my Mikrotik at the bottom then ping to the switch start again, but only last 3-5 seconds.

You switch is more then likely damaged, you will need to RMA it do determine if it is a failure or damage but most often this is damage from a surge or ground potential shift.

But you should first try a cold reboot / power cycle and upgrade to latest firmware which is v1.4.3.rc6
