We had a WS-150-AC that was at a client's site with a locoM5 and computer connected to the switch. The switch was plugged into a UPS and everything has been working fine since deployed until the other day when the switch suddenly stopped passing traffic. We replaced the switch and brought the other back for bench testing.
Upon powering on the switch, all port lights are "twinkling" rapidly. Port 1, which had 24V power for the loco, still outputs power. We ran through the reset process (hold down reset, apply power, release after 45 seconds, wait 5 minutes, etc), but it won't reset. Also tried via serial the newly released recovery method, which appears to complete (100% on progress bar, but dotted line continues to run across status windows). No data is being passed across ethernet ports that we can see.
What could be the cause of this? Is there a resolution?

Well let's do some investigation. If you can use the recovery tool then you must have a serial connection. Try going opening putty (or whatever emulator you like) and run the commands:
