I wish I could give you an answer but I can not unless I am testing it.
For all we know the radio you took out of service was the issue?

Agreed, we need to test it.
I tried to enter the WebGUI (after what I did in my last post), and the switch stopped reacting. I was hoping to delay the replacing till next week, but I am now configuring a WS-10-250-AC, and will replace within a hour.
Testing will probably be next week, I will keep you updated.

I always update the switches at my home as soon as possible, let it run for a couple of days, and then I update them at the live location.
So this one was running the latest version, v1.5.0
The WS-10-250-AC is running fine, I also connected the Unifi AP AC Lite again. No issues so far.
Bench test will be next week.
