I have just replaced it now with an ordinary unmanaged switch, if I still get problems tonight it is not your switch at fault.
If I do not get any problems tonight then it is your switch.

Possibly but IMHO that is overly simplistic reasoning. In my long networking career I have found that combinations of hardware can sometimes exacerbate issues in other places.

I am only using it as a basic POE switch nothing complicated, I have put the original adapters back and for the time being an unmanaged switch, no problems with the devices getting DHCP addresses.
Customers will soon let me know tonight if there is still a problem on the network.

Well the DHCP issue is fixed with v1.3.0rcX, I have asked Eric to backport the fix to 1.2.X but like any programmer he wants to EOL v1.2.x and move to v1.3.X but I have not had enough time to work on testing it enough to call it stable however all the DC switches we are shipping out will only work with v1.3.X so....

Well I do not know what to say we have had a glitch free night after replacing the WS-12-250A with the Ubiquiti adapters and an unmanaged Netgear switch, so I am pretty confident the problem is with the switch.
When I fitted the switch originally ( having updated firmware on the bench and setup the ports for the correct voltage ), with unit off, I took the cables out of the Ubiquiti adapters and connected them to the switch ports, I then powered on the switch, I could not log in to switch because it did not get an IP address, DHCP not working, I set laptop IP with a static so I could log in and change IP from default to a static in local range.
Everything seemed OK until the evening when under load and the ports started dropping for 90secs, updated firmware to 1.2.4, ports still dropping when under load, tried un-ticking loop protection, OK with light traffic, still dropping data under load.
