New WS-12-250-AC Hardware issues?

Hi,
Brand new Netonix WS-12-250-AC. After a few days working it started acting up all of a sudden.
In log the following show up:
Apr 10 02:40:29 switch[2782]: Detected cold boot
Apr 10 02:40:34 switch[2746]: 24V is out of range (4.3V)
Apr 10 02:40:39 switch[2746]: Warning: expected 1 fans but found 0
The switch powers only an AF24 nothing else.
Attached screenshot.
Please assist.

Either the unit failed or was damaged.
You will have to RMA it to determine which it is.
Describe the site:
Tower:
Is the the tower grounded?
Equipment box location:
Where you get AC power from:
Is the tower grounded?
Are the tower ground rods bonded to the electrical service ground rods?

This is not on a tower, but indoors inside a house - power is fed by a UK wall socket (plug properly grounded and fused)
House is properly grounded as checked by simple ground tester.
The fan was really noisy since first boot and then it failed along with the 24v supply as it would seem.

Yep, like he said -- will need to RMA to restore function. The fan is powered by the 24v supply, and I have seen a few odd cases where the 24v circuit will cease to function, or be dragged down externally by failing hardware.
If all you'll ever power is an AF24, (meaning no 24v devices), and your temperatures seem stable (bearing in mind that the system is rated to operate at 55C ambient, and tested over 90), and everything is stable, it's up to you if you want to return your switch, since everything still works in your application (aside from some power and fan related log entries).
If you would prefer to RMA, I will absolutely personally ensure that your switch leaves here in electronically new condition, and the procedure to do so can be found here:

Hi,
We would like to avoid having to re-pay for import tax and fees for the replacement netonix. This is why we purchased it from the EU in the first place.
Is there any way you can assist us with replacement parts for some DIY on our side?

If the unit is failed and repaired under warranty we ship it under and HTS Code that should should NOT incur any Tax.
If the unit is damaged and repaired you should only be charged Tax on the repair fee.

We're generally pretty flexible, but component-level, this repair is beyond the scope of most shops' equipment. If you have an SMT rework station and a technician with whom to use it, we can speak further on that subject.
board-level can be done yourself, what would happen is you would purchase a replacement board/fan and return yours; after we examined yours we would of course refund the purchase price if warranty would normally cover repair. If not covered, however, your total cost would be ~$75 higher than had you returned the unit for repair.
but, like Chris said: we do ship repaired switches under an HTS code which shouldn't incur any duties, taxes, or tariffs, so there's that, as well.
