WS-8-150-AC: All interfaces go down, power to radios remains

Hi all,
We had situation where a WS-8-150-AC with ~75watts of load (thanks mostly the 1 x WS-6-MINI on port 1) would loose Ethernet line protocol on all ports however anything relying on the switch for power remains up (the mini remained reachable via a radio connected to it).
Some observations or other notes:
No other AC devices at the site were affected;
All revenue ports lost line protocol, including a port to a router and the port to the WS-6-MINI;
All revenue ports did occasionally come up for 1-2 seconds then go back down again, not really long enough to forward traffic but enough to auto negotiate;
Issue resolved itself for a very short period of time during business hours with a reboot;
Issue resolved *seemed* to resolve itself when removing the WS-6-MINI - this state wasn't tested for any length period of time and could be a red herring;
Issue didn't manifest itself overnight during low throughput and reoccurred again during business hours;
Switch was in production with this power draw for over a month without issue;
Traffic throughput has increased as of 4 days ago, only an issue as of 2 days ago in the evening;
Traffic throughput peak was <100mbps;
The issue persisted through multiple firmware versions - 1.4.6, 1.4.7rc11, 1.4.7rc11 and 1.4.7rc21;
Switch was originally running version 1.4.7rc11 when we put it into production (as is our entire Netonix fleet);
Switch was upgraded and downgraded respectively troubleshooting this issue;
Issue persisted with and without STP/RSTP enabled globally and on a port level.
We put in WS-8-150-DC (Rev F) running off a 12v battery and charger replacing the AC version so far with no issues.
Has anybody seen this behavior before? Originally I thought it might be a power supply issue, but running at half capacity on the AC switch didn't jive especially once I learned that all radios kept power.
