I bought a used WS-12-250A, Revision C. It reports "switch[772]: Warning: expected 2 fans but found 0", yet both fans are running. Is this a firmware issue? Or is it a hardware failure? Or could this be due to the switch having a pair of ADDA model AD0424UB-C56 fans?

What, they are not factory fans?

No this is not a firmware issue but you should have the latest firmware loaded which is v1.4.7 or v1.4.8rcX

You could always RMA it for a check up.
