WS-12-250-AC rebooting / dead

Cleaning up my office I realized I've got a WS-12-250 that needs to be RMA'd.
My initial notes from July (hence the old FW ) :
Symptoms: This was a spare switch still in the box.
Took it out yesterday to replace a WS-8 (needed more ports), upgraded to 1.4.7, configured, and put into production.
This morning I noticed it was rebooting. Acts like normal power up, but cuts out, fan sounds sick, eventually boots, then starts all over again.
Defaulted via button, now the fan comes on but doesn't boot any further - no lights, no ping.

Not sure if this is one with a cap issue or not.
Three of the ports were set to 24V and one was 48V
Put the WS-8 back and it has been working fine since.... now I need more ports again.
