WS-26-400-IDC not responding after reboot

We had a WS-26-400-IDC with 1 Ubiquiti AF24HD and 17 Ubiquiti 24v 5Ghz devices with 4 VLANs. Yesterday we had a issue with our switch stopped all traffic. We were able access the switch but none of devices attached. We attempted to reboot the switch. The switch was pingable but we were not able to access the web gui or access via SSH. We dispatched someone to go onsite. They manually power cycled the unit. We did get access back up but were only able to access 4 devices on the entire switch. We had several more devices on the same VLAN that would not respond even after port bounces. We made a decision to replace the unit. We had this problem previously about a month ago but at that time a simple on site reboot resolved the issue. We are currently running 1.5.1 firmware.

NOTE: WE REMOVED v1.5.1 FROM CIRCULATION AS THERE IS A BAD BUG WITH STP INTERACTING WITH LAGS.
PLEASE USE V1.5.2rc4 OR NEWER. ALSO YOU MUST USE v1.5.2rc4 OR NEWER WITH ANY RECENTLY PURCHASED SWITCHES AS YOU NEED THIS FIRMWARE TO PROPERLY DRIVE THE NEW FANS.

I would take the unit back to the shop

Upgrade the firmware
