SFP: Fault on SFP4, resetting...

My log is filling up with the message:
"SFP: Fault on SFP4, resetting..."
Only one problem, it's a WS-24-400B (Firmware 1.5.5, board revision D) which only has two SFP ports. And there's nothing plugged into either of them anyway.
Anybody have any ideas?

I've always seen this on some of the older 24 ports. I have about 6 that do this. Running 1.5.5, has been doing it for years so its not this image.

Never stops. None of the ethernet ports are blown and the 2 SFP ports do work.
