I have a WS-12-250-DC that reboots periodically. Now it seems to have a fan problem

Log says
Mar 4 02:29:12 switch[799]: Fan 1 is failing (442 RPM, cooling level 255, board temp 55 C)
Mar 4 02:37:02 switch[799]: Fan 1 is now working
Mar 4 06:58:26 switch[799]: Fan 1 is failing (442 RPM, cooling level 255, board temp 55 C)
Mar 4 07:05:08 switch[799]: Fan 1 is now working

Currently the status page says it is doing 3600 RPM or so.

I assume the fan can be replaced. Is there any info on how to do that?

It just takes a Phillips screwdriver and a little patience to open the case. The fan plugs into the top of the PCB.

The only suggestion if you have not done this before is to support the back of the new fan so it is flat against the inside of the case on both screws so that they can self-tap nice perpendicular holes into the plastic. :)

Edit: OHhhh ... and it might be good to suggest that you keep track of which socket the old fan pigtail plugs into, because there may be an identical socket which is used for other purposes near it.

If you have ever seen the typical case cooling fan, there are arrows molded into the plastic frame to indicate the direction of airflow, that makes it easy to match the old one even without power to the fan.
