Can't Set Password

I have a WS-12-250-AC that no matter how many times I try to set the administrative password it never sets. It tells me it did, but when I go back to log into it it's back to admin/admin.

Any idea what might be going on here and how to fix it?

Not certain off the top of my head.

Might want to check the logs to see if there is a clue there.

I have a the same model in my lab running 1.5.9rc1, after changing the password I log out and log back in with the new password and I can see this in the logs:

Is there anything in there that looks similar?
