I have a spare unit that is configured as backup and ready to go - I might try installing it.
Do you think a hard reboot might help?

You are probably right in that it is not the switch, but it is strange that the switch is completely inaccessible from VLAN1 during an outage.

Anything is possible, can not hurt to do the following:

1) Upgrade to v1.3.9tc14 - I do not think this will solve it but it is needed to be sure
2) Cold reboot but this is not something that is indicative to something a cold reboot will fix.
3) Swap with spare switch to be sure this is not a bad unit.

Beyond that you need to do some digging.

Firmware upgrade went smooth and may have led me to the potential culprit. Upon loading of the new firmware, Port1 produced an error message and would not power up the connected AF24. I unplugged the device and turned the port back on. When the device was plugged in, an outage occurred...unplug it and things returned to normal.
That being said, I believe that could be the issue as an outage has not occurred since being unplugged. Anyone else witnessed this before? Other thoughts?

You need to be a little more descriptive.

What error message?
What is plugged in there?

Maybe provide a few lines before and after the event.

You did upgrade this right and what version were you upgrading from?

There was a bug SEVERAL versions ago where the first POE port would be disabled on reboot but that was many versions ago like v1.3.3 and v1.3.5

After upgrading to v1.3.8 and or preferably v1.3.9rc15 and a reboot and that is fixed.
