WS-12-250-DC - Port Voltage Lights

I have a new WS-12-250-DC on the bench. While provisioning devices, the power status lights stopped changing as ports were turned on/off. Lights that were already on, stayed on until the switch was rebooted.
After reboot, all power LEDs stayed off.

Things that were tried:

- Complete power down
- Re-flashed to 1.5.5
- Re-flashed to 1.5.6
- Factory defaulted unit

Otherwise, switch operates properly - the proper voltage is provided to the ports (as reflected by the GUI) & ethernet ports negotiate properly, with no errors.

Is this possibly a loose cable inside?

sorry, no cables for LEDs..sounds like you need to RMA unit...
